Alpha-methylacyl-CoA racemase (AMACR), also known as P504S, is a mitochondrial and peroxisomal enzyme involved in lipid metabolism, particularly the β-oxidation of branched-chain fatty acids and bile acid intermediates. Its gene, located on chromosome 5p13. encodes a protein that catalyzes the conversion of (2R)-methyl-branched fatty acyl-CoA esters to their (2S)-stereoisomers, a critical step in energy production pathways.

AMACR gained prominence in oncology due to its overexpression in various cancers, most notably prostate adenocarcinoma. Studies in the early 2000s identified AMACR as a sensitive immunohistochemical marker for prostate cancer diagnosis, helping distinguish malignant glands from benign proliferative lesions in biopsy specimens.
