背景信息

**Background of TPO Antibodies**

Thyroid peroxidase (TPO) antibodies are autoantibodies targeting thyroid peroxidase, an enzyme critical for thyroid hormone synthesis. TPO catalyzes iodide oxidation and thyroglobulin iodination, essential steps in producing thyroxine (T4) and triiodothyronine (T3). In autoimmune thyroid disorders, the immune system mistakenly identifies TPO as foreign, triggering antibody production.

TPO antibodies are hallmark biomarkers of autoimmune thyroid diseases (AITDs), including Hashimoto’s thyroiditis (chronic lymphocytic thyroiditis) and Graves’ disease. Elevated TPO antibodies are detected in ~90% of Hashimoto’s cases, correlating with thyroid inflammation and eventual hypothyroidism. In Graves’ disease, their presence often coexists with TSH receptor antibodies, indicating broader autoimmune activity. These antibodies also aid in diagnosing postpartum thyroiditis, silent thyroiditis, and predicting thyroid dysfunction risk in euthyroid individuals.

Clinically, TPO antibody testing helps differentiate autoimmune causes from other thyroid disorders. Their levels may correlate with disease severity but do not consistently predict progression. Around 10-15% of healthy individuals may have low TPO antibody titers, often linked to genetic predisposition or subclinical autoimmunity.

Research highlights their role in understanding AITD pathogenesis, particularly T-cell-mediated thyroid destruction. Additionally, TPO antibodies are associated with non-thyroid autoimmune conditions (e.g., type 1 diabetes, rheumatoid arthritis), underscoring systemic immune dysregulation. Despite diagnostic utility, their exact pathogenic mechanisms remain partially unclear, warranting further investigation.
