Cytokeratin 7 (CK7) is a member of the cytokeratin family of intermediate filament proteins, which are essential structural components of epithelial cells. These proteins are classified into acidic Type I and basic Type II keratins, forming heteropolymers to maintain cellular integrity. CK7. a Type II keratin with a molecular weight of approximately 54 kDa, is expressed in specific epithelial tissues, including glandular and ductal epithelia of the respiratory, biliary, urinary tract, and endometrium.
In diagnostic pathology, CK7 antibodies are widely used in immunohistochemistry (IHC) to identify epithelial origin and differentiate between carcinomas. For instance, CK7 positivity is common in adenocarcinomas of the lung, breast, and ovary, while it is typically absent in colorectal, prostate, and hepatocellular carcinomas. This marker is often paired with CK20 to refine tumor classification (e.g., CK7+/CK20– in lung adenocarcinoma vs. CK7–/CK20+ in colorectal carcinoma).
CK7 antibodies also aid in detecting metastatic tumors of unknown primary origin by highlighting tissue-specific expression patterns. However, interpretation requires caution, as staining intensity and distribution may vary due to technical factors or tumor heterogeneity. Clinically, CK7 remains a cornerstone in differential diagnosis, though results should always correlate with clinical context and additional biomarkers.
