**Background of SSB Antibodies**
SSB antibodies, also known as anti-La antibodies, are autoantibodies targeting the SSB (Sjögren’s syndrome type B) antigen, a 48-kDa phosphoprotein associated with RNA polymerase III-transcribed RNAs. This antigen plays a role in RNA processing and quality control, particularly in the stabilization of small RNA molecules like tRNA precursors. SSB antibodies are clinically significant in autoimmune disorders, most notably Sjögren’s syndrome (SS) and systemic lupus erythematosus (SLE).
These antibodies are detected in approximately 30-60% of primary SS patients, often coexisting with anti-SSA/Ro antibodies. Their presence is associated with more severe exocrine gland dysfunction, including xerostomia and keratoconjunctivitis sicca. In SLE, SSB antibodies are less common (10-15% of cases) and may correlate with specific manifestations like cutaneous involvement or neonatal lupus, particularly congenital heart block in offspring of antibody-positive mothers.
Diagnostically, SSB antibodies are part of the serological criteria for SS and aid in differentiating primary from secondary SS. Their detection, typically via ELISA or immunoblot, complements clinical evaluation. While their exact pathogenic role remains unclear, SSB antibodies are thought to arise from immune dysregulation triggered by viral infections (e.g., Epstein-Barr virus) or genetic factors (e.g., HLA-DR3 haplotype). Research continues to explore their molecular mechanisms and prognostic implications in autoimmune diseases.
