背景信息

Recoverin antibodies are autoantibodies targeting recoverin, a calcium-binding protein expressed primarily in retinal photoreceptor cells. Discovered in the 1990s, recoverin plays a role in modulating phototransduction by regulating rhodopsin kinase activity in response to calcium levels. Its association with autoimmune retinopathy emerged when recoverin was identified as a key antigen in cancer-associated retinopathy (CAR), a paraneoplastic syndrome often linked to small-cell lung carcinoma or other malignancies. In CAR, immune cross-reactivity between tumor antigens and retinal proteins triggers antibody-mediated attacks on photoreceptors, leading to progressive vision loss, photophobia, and night blindness. Recoverin antibodies are also detected in melanoma-associated retinopathy (MAR) and some non-paraneoplastic autoimmune retinopathies. Structurally, recoverin contains EF-hand calcium-binding motifs, and its immunogenicity may relate to calcium-dependent conformational changes exposing cryptic epitopes. Diagnosis involves detecting recoverin antibodies in serum or ocular fluids via Western blot, ELISA, or cell-based assays. While their pathogenic mechanism remains debated, these antibodies are thought to induce photoreceptor apoptosis through calcium dysregulation or direct cellular entry. Clinical management focuses on immunosuppression, tumor treatment (if applicable), and plasma exchange. Research continues to explore recoverin's role in retinal physiology and its interplay with autoimmune responses.
