The GAD67 antibody is an autoantibody targeting glutamic acid decarboxylase 67 (GAD67), a key enzyme that catalyzes the conversion of glutamate to gamma-aminobutyric acid (GABA), the primary inhibitory neurotransmitter in the central nervous system. GAD67. one of two GAD isoforms (GAD65 and GAD67), is predominantly expressed in GABAergic neurons and pancreatic β-cells. Autoantibodies against GAD67 are strongly associated with autoimmune neurological disorders, including stiff-person syndrome (SPS), cerebellar ataxia, and epilepsy, as well as type 1 diabetes (T1D). In SPS, these antibodies are thought to impair GABA synthesis, leading to hyperexcitability of motor neurons and characteristic muscle rigidity. While GAD65 antibodies are more commonly linked to T1D, GAD67 antibodies are occasionally detected in overlapping autoimmune conditions.
