Thrombomodulin (TM), also known as CD141. is a transmembrane glycoprotein expressed predominantly on vascular endothelial cells, with additional presence on monocytes, dendritic cells, and certain epithelial tissues. It plays a critical role in regulating coagulation and inflammation. Structurally, TM consists of an N-terminal lectin-like domain, six epidermal growth factor (EGF)-like repeats, a serine/threonine-rich region, a transmembrane domain, and a short cytoplasmic tail. Its fifth and sixth EGF-like domains bind thrombin to form a complex that activates protein C, initiating an anticoagulant pathway by inactivating factors Va and VIIIa. Beyond coagulation, TM modulates inflammation by interacting with thrombin-activatable fibrinolysis inhibitor (TAFI) and high-mobility group box 1 (HMGB1) protein.
